All local Health Departments are responsible for vital records for their specific county.  We only have birth and death records for Montgomery County.  You will need to contact the Health Department in the county of the state you were born in to retrieve a record. Montgomery County Marriage certificates and information can be found at the County Clerk's Office located in the Montgomery County Court House.  You may come into our office Monday through Friday from 8:00 am to 4:00 pm to request a certified copy of a certificate.  

Online options are available for requesting a record from the Health Department.  Click here to access the online Birth or Death Certificate Application. Simply fill it out the form online and then print a copy.  You can then scan and email, fax, or mail a copy of this form along with a copy of your driver's license or other identifying information to our office.  Online payments are available through the following link Vital Records Payment Center or you can send in a money order for $10.00 for each copy requested, made payable to MCHD.

A FEW FACTS ABOUT BIRTH CERTIFICATES

Certified Birth Certificates are issued in the County that the person was born. To be a legal copy the birth certificate is printed on specialty paper that is then embossed with the seal of the Health Officer.  The Hospital Birth Certificate is a complimentary gift copy and cannot be used as a legal document. Montgomery County started keeping birth and death records in 1882. You can also obtain birth certificates from the Indiana State Dept. of Health, Vital Records Division.

Birth Certificates are used for proof of identity - you may need a certified copy of your birth certificate for any of the following reasons:

  1. To obtain a State ID or Driver's License from BMV;
  2. To obtain employment;
  3. For a Passport;
  4. To go on a cruise, or travel to Canada or Mexico;
  5. For Social Security Administration; and/or
  6. Registration for school

Since your birth certificate has vital information contained on it, you should Never carry it in your wallet or purse, in case you lose it or it gets stolen, as we are becoming more and more aware that "Identity Theft" is on the rise at an alarming rate.

Death Certificates are used to prove someone has passed on. Who would need proof?

  1. Insurance Companies;
  2. Banks;
  3. Attorney's;
  4. Credit Card Companies; and
  5. The Social Security Administration Offices.
